65 year old female with uncontrolled diabetes

 A 65 year old female presented to the department on 15th august with chief complaints of decreases Appetite ,weakness and vomiting since 6 days.

HOPI : She was having the same complaint since 6 months.She had vomiting immediately after food intake.Vomiting was non bilious and non projectile.  She also complained of onset of fever every evening and would last for 4 -5 days and would subside after taking paracetamol.She was admitted 6 months ago with complaints of fever and urinary incontinence.On investigating it was found that her serum creatinine levels were increased and was diagnosed with kidney disease. She also complained of hypersomnia since 6 months.There is significant weight loss.

Past History: She was diagnosed with type 2 Diabetes mellitus 20 years ago.And she takes insulin injection twice a day.

                     She was also diagnosed with hypertension 6 months ago.

Family history: No significant family history

Personal History: Mixed diet

                               Decreased Appetite

                              Hypersomnia

                             Polyuria since 6 months

                            Bowels -Irregular  since 6 months

                            She has addiction eating pan 

  

Drug history: No significant drug history

General Examination 

Semiconscious and coherent

Poorly built and nourished

Pallor -present 

No cyanosis

No icterus

No lymphadenopathy

No clubbing of fingers

No pedal edema

Vitals:

Temperature:Afebrile

Pulse rate : 72 bpm

Respiratory Rate : 20 cpm





BP: 130/80 mmhg

Spo2: 97% on RA

GRBS : -128 mg/dl

Diagnosis: uncontrolled type 2 diabetes

A 55 year old female with abdominal distention

A 55 year old female presented to the OP with chief complaints of abdominal distention and shortness of breath since 15 days    HOPI :           The patient was completely normal 1 month ago. since 15 days she was complaining of abdominal distention which was  with pain which  subsided on taking medication. She was having episodes of mild fever once in a while which subsided when she had Dolo.She also had frequent episodes of shortness of breath and also faced difficulty in walking. The patient also complained of bilateral edema of both the lower extremities.
